State of autonomous action, 2026 Q4

2026-10-01 to 2026-12-31Quarter to date: numbers as of 2026-11-01Draft, unsigned
Draft. The figures are computed from the Register; the narrative is the agent's draft until a named person signs the report. Quote the figures with the Register's name; quote the narrative only once signed.

Summary

This quarter the Register holds 25 cases, 25 of them added since 2026-10-01. In the machine cases the controls that fail most are about perceiving the world; in the software-agent cases they are about authority. These are counts of the Register's curated cases, not estimates for autonomous systems in general.

Cases added to the Register this quarter

The Register began this quarter, so this list includes events from earlier years (Knight Capital is from 2012). It counts when a case entered the Register, not when the event happened.

What these numbers can and cannot say

Every figure in this report counts the Register's included cases. The cases are curated, not sampled, so no figure estimates how often anything happens to autonomous systems in general, and differences between groups of cases are exploratory. Researchers can find the tests behind each comparison in the appendix to the Patterns page.

Findings and their labels

FindingCountLabel
Where one company built the automation and someone else ran it, the builder has been fined or found liable in two cases, both after deaths. No company has yet been fined or found liable as the maker of an AI agent, chatbot or AI tool.2 of 25 casesdocumented
The actor did what it was built to do, with a picture of the world that was wrong.9 of 25 casesrecurring
The actor held authority the task never needed.8 of 25 casesrecurring
The constraint was an instruction, not a mechanism.4 of 25 casesrecurring
Of the cases that ended without harm, most were averted by an outside researcher, not by a control the operator had in place.7 of 10 casesrecurring
The warning existed before the event, and nobody treated it as one.5 of 25 casesrecurring
Recovery sat inside the failure domain.2 of 25 casesdocumented
The actor's own account was the only witness, or no one could name the actor at all.4 of 25 casesdocumented
Two courts have said delegation does not move responsibility off the delegator, and may extend it to the vendor.2 of 25 casesdocumented
